Nurse Practitioner
JOB SUMMARY
The Nurse Practitioner is directly responsible for obtaining the client's thorough health history, identifying, and recognizing the severity of client's chief complaint, performing a complete physical exam, diagnosing condition(s), managing immediate health care needs, implementing a medical management plan that is appropriate/suitable to client's needs, and complete documentation of the above in client's chart. The Nurse Practitioner is authorized to provide medical care without the direct observation and supervision of a Physician. However, the Nurse Practitioner is responsible for consulting a Physician for any situation(s) that is deemed to be beyond the scope of practice/experience of the
Nurse Practitioner. Participates in a team-based approach to care in the Patient Centered Medical Home (PCMH). Participates in activities related to Quality Improvement.
ESSENTIAL JOB DUTIES

Manages patient medical care (according to Nurse Practitioner Protocol) including diagnosis, treatment, and follow-up, and accurately documents above in medical records.
Participates in educational aspect of all clients which includes individual or group counseling and distribution of educational materials.
Maintains complete responsibility for client's health care regarding tracking of all referrals, mammograms, pap smears, immunizations, laboratory reports, X-rays, and prescribing authority.
Provides holistic patient care through collaboration of other members of interdisciplinary health care team: Physician, Dietician, Social Worker, MIHP, Nurses, WIC, and other internal/external sources.
Attend monthly provider meetings.

MARGINAL JOB DUTIES
Designs and implements strategies to effect change of current health care system as appropriate, participates in task forces, and functions as an intermediary between nursing staff and Physicians to reinforce or implement change.
Participates in the continuing education of nursing staff, nurse practitioner students, medical students, interns, residents and other students.
Performs other functions related to health center as directed by GLBHC and/or Medical Director.
JOB SPECIFICATIONS
Education: Evidence of completion from either an accredited Nurse Practitioner Certificate program or Master's Degree Nurse Practitioner Program which conforms to Board of Registered Nursing standards.
Licensure: N. (State of Michigan), and Specialty Certification by State of Michigan Board of Registered Nursing and/or Specialty Certification through the ANA as a Nurse Practitioner.
Experience: Previous experience in FQHC setting preferred.
Skills: Participates in ongoing education as required for maintenance of Nurse Practitioner certification. Bilingual (English/Spanish) preferred.
Interpersonal Ability to communicate effectively with a diverse population.
Physical Effort: Medium.
Hours of Work: 40 hours per week, or as necessary to complete the job. Flexible and varied.
Travel: Local travel except for conferences. Reimbursement in accordance with contract and GLBHC policy.
